Q: I HAVE RECEIVED AN ITEM AND I THINK IT'S FAULTY, WHAT DO I DO?

A: First of all, we are sorry to hear this has happened! We are more than happy to assist you with this fault, please email your order details along with images of your item to customerservice@form-active.com and we will help you out.

 

Q: ARE YOUR ITEMS ETHICALLY MADE?

A: FORM ACTIVE is proudly ethical in the way our pieces are manufactured. We have a great relationship with our suppliers and factory employees and are regularly in contact with them regarding production. They are highly skilled professionals, paid correct wages and work in a clean, modern environment with top of the line machinery.

 

Q: HOW DO I WASH MY FORM PIECES? 

A: Our pieces are made from beautiful quality fabric. To keep it in it's best condition we always recommend for our customers to look after their garments with the highest level of care.

Either hand wash or pop your pieces in a delicates bag into the washing machine on a cold wash cycle with like colours. Hang your pieces in the shade to dry.

 

Q: WHERE IS FORM BASED?

A: FORM Active is an Australian based label and our warehouse and offices are located in Newcastle, NSW.

 

Q: I PLACED AN ORDER ONLINE BUT HAVE CHANGED MY MIND, CAN I CANCEL FOR A REFUND?

A: FORM ACTIVE does not offer refunds for change of mind and all sales are final. If you are no longer wanting your pieces, feel free to return them for a store credit here: https://360.postco.co/form-active
